But for some Canadians, the decision to leave the country isn&#8217;t out of a desire to get away &#8212; it\u2019s a medical necessity.<\/span><\/p>\n<p><span style=\"font-size: medium;\">There are a variety of factors pushing people overseas for medical treatment: Canada can have long waiting lists, high expenses for procedures that aren\u2019t fully covered by provincial healthcare plans, and it lacks\u00a0many experimental treatments.<\/span><\/p>\n<p><span style=\"font-size: medium;\">Often, these Canadians turn to some of the many Internet sites that exist\u00a0to get Canadians medical care outside of the country. These websites offer consultations, and advertise to arrange the best private medical care that works for their clients, for a cost.<\/span> <span style=\"font-size: medium;\"><span style=\"font-weight: 400;\">It\u2019s all part of the business of medical tourism, which is<\/span>\u00a0the process of travelling outside of the country to seek medical care.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">The websites run by medical tourism agencies or facilitators advertise to a specific clientele: people interested in travelling outside their home country for treatments or surgeries, otherwise known as medical tourists.<\/span> <span style=\"font-weight: 400; font-size: medium;\">These travellers can seek out a variety of treatments abroad &#8212; everything from <a href=\"http:\/\/www.holidaydental.ca\/\">dental work<\/a> to <a href=\"http:\/\/www.medretreat.com\/procedures\/treatment_types\/cosmetic.html\">plastic surgery<\/a>, and even experimental <a href=\"https:\/\/hsci.harvard.edu\/stem-cell-tourism\">stem cell treatments<\/a>.<\/span><\/p>\n<p>[\/et_pb_text][et_pb_image src=&#8221;https:\/\/images.unsplash.com\/photo-1514416205405-075ab2f15964?ixlib=rb-0.3.5&amp;s=fc6dce78cab7f98ab5c5309b1d70c1f2&amp;auto=format&amp;fit=crop&amp;w=750&amp;q=80&#8243; _builder_version=&#8221;3.0.89&#8243; custom_margin=&#8221;||-2px|&#8221; \/][et_pb_text _builder_version=&#8221;3.0.89&#8243; background_layout=&#8221;light&#8221;]<\/p>\n<p><strong>Patients may travel abroad for a variety of different surgeries, including cardiac surgery. Image by Piron Guillaume, via <a href=\"https:\/\/unsplash.com\/photos\/vNFHg0J0wRs\">Unsplash<\/a>.<\/strong><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">Becoming a medical tourist is simple: people arrange their out-of-country treatment and travel, either through these medical tourism agencies or on their own.<\/span> <span style=\"font-weight: 400; font-size: medium;\">In Canada, there is no government intervention or regulation for medical tourism.<\/span><\/p>\n<p><span style=\"font-size: medium;\">\u201cIf I want to hop on a plane to Mexico tomorrow and get bariatric surgery, there\u2019s really no limitation on that, no tracking of the numbers, no regulated follow up care,<span style=\"font-weight: 400;\">\u201d said Jeremy Snyder, a health sciences professor from Simon Fraser University in Burnaby, B.C., in a phone interview<\/span><\/span> <span style=\"font-size: medium;\">It\u2019s a well-worn myth that universal health care is one of the pillars of Canadian identity, along with hockey and maple syrup. Many Canadians take pride in the country\u2019s publicly funded health-care system.<\/span><\/p>\n<h4><span style=\"font-size: medium;\">But the number suggest a growing counter-narrative. Increasingly, Canadians are voting with their passports and deciding to seek treatment elsewhere.<\/span><\/h4>\n<p>[\/et_pb_text][et_pb_testimonial author=&#8221;Jeremy Snyder&#8221; background_layout=&#8221;light&#8221; _builder_version=&#8221;3.0.89&#8243; custom_margin=&#8221;75px||75px|&#8221;]<\/p>\n<p><span style=\"font-size: x-large;\"><strong>If I want to hop on a plane to Mexico tomorrow and get bariatric surgery, there\u2019s really no limitation on that, no tracking of the numbers, no regulated follow up care.<\/strong><\/span><\/p>\n<p>[\/et_pb_testimonial][et_pb_text _builder_version=&#8221;3.0.89&#8243;]<\/p>\n<p><span style=\"font-size: medium;\">A <a href=\"https:\/\/www.fraserinstitute.org\/sites\/default\/files\/leaving-canada-for-medical-care-2017.pdf\">2017 study from the Fraser Institute<\/a> shows that 2016 marked a sharp increase in Canadians leaving the country for medical care.\u00a0<\/span><span style=\"font-size: medium;\">In <a href=\"https:\/\/www.fraserinstitute.org\/studies\/leaving-canada-for-medical-care-2015\">2015<\/a>, nearly 46,000 Canadians were medical tourists, whereas in 2016, this number increased to nearly 64,000.<\/span> <span style=\"font-size: medium;\">Since the think-tank\u2019s study began in 2014, the number of Canadian medical tourists has been increasing steadily. This increase has raised ethical questions &#8212; not only concerning Canadians travelling abroad, but also surrounding the potential impacts on destination countries and the Canadian health-care system.<\/span><span style=\"font-size: 14px;\">\u00a0<\/span><\/p>\n<p>[\/et_pb_text][et_pb_image src=&#8221;https:\/\/cusjc.ca\/catalyst\/wp-content\/uploads\/2018\/03\/Screenshot-2018-03-15-18.39.13.png&#8221; _builder_version=&#8221;3.0.89&#8243; max_width=&#8221;53%&#8221; custom_margin=&#8221;25px||45px|&#8221; \/][et_pb_text _builder_version=&#8221;3.0.89&#8243; background_layout=&#8221;light&#8221;]<\/p>\n<h3><strong>An Enduring Phenomenon<\/strong><\/h3>\n<p><span style=\"font-size: medium;\"><span style=\"font-weight: 400;\">Medical tourism isn\u2019t a new. People have been going to different places for care since the<\/span><a href=\"http:\/\/content.time.com\/time\/health\/article\/0,8599,1861919,00.html\"> <span style=\"font-weight: 400;\">ancient Greeks and Romans<\/span><\/a><span style=\"font-weight: 400;\"> &#8212; but this has mostly been people going to more economically developed countries.<\/span><\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">As Snyder explains, developing countries such as India, Thailand, and Mexico have only recently become popular destinations.<\/span> <span style=\"font-size: medium;\"><span style=\"font-weight: 400;\">In some cases, medical tourism means people coming from other countries to Canada for treatment. <\/span><span style=\"font-weight: 400;\">But a large number of Canadians also travel abroad for medical care.<\/span><\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">The reasons Canadians choose to become medical tourists vary, but Snyder says it\u2019s mostly due to cost, care that&#8217;s unavailable or illegal in Canada, or wait times.<\/span> <span style=\"font-size: medium;\">Raywat Deonandan, a professor of health sciences at the University of Ottawa, says that medical tourists might target countries with certain specializations.<\/span> <span style=\"font-weight: 400; font-size: medium;\">Brazil, for example, specializes in reconstructive surgery, whereas Turkey is best with eye procedures, he says.<\/span><\/p>\n<p>[\/et_pb_text][et_pb_code _builder_version=&#8221;3.0.89&#8243; custom_margin=&#8221;15px||-3px|&#8221;]&lt;iframe width=&#8221;650&#8243; height=&#8221;450&#8243; scrolling=&#8221;no&#8221; frameborder=&#8221;no&#8221; src=&#8221;https:\/\/fusiontables.google.com\/embedviz?q=select+col0+from+1RcXcWImoCqhw4tLzRCAOEOkZwUG75l1bohzkF9Kd&amp;viz=MAP&amp;h=false&amp;lat=21.27032635483846&amp;lng=66.09848899999997&amp;t=1&amp;z=2&amp;l=col0&amp;y=2&amp;tmplt=2&amp;hml=ONE_COL_LAT_LNG&#8221;&gt;&lt;\/iframe&gt;[\/et_pb_code][et_pb_text _builder_version=&#8221;3.0.89&#8243; background_layout=&#8221;light&#8221;]<\/p>\n<p><strong>An interactive map showing popular medical tourism destinations, with a list of their specifications. Visualization by Kristian Kelly. <\/strong><\/p>\n<p><span style=\"font-size: medium;\">Deonandan says it\u2019s a misconception that medical tourists are rich people who go to other countries to relax on the beach and buy care that lower-income people can\u2019t afford.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">\u201cOften, it\u2019s desperate people who are going into debt to get this thing done that they need,\u201d he said in a video response to questions.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">Deonandan says there are generally two types of medical tourists: those who are price-sensitive seek less expensive treatment, whereas those who are quality-sensitive dish out more cash to get superior or unavailable treatment, or to avoid wait times.<\/span><\/p>\n<p>[\/et_pb_text][et_pb_image src=&#8221;https:\/\/cusjc.ca\/catalyst\/wp-content\/uploads\/2018\/03\/Benchmarks.jpg&#8221; _builder_version=&#8221;3.0.89&#8243; max_width=&#8221;75%&#8221; custom_margin=&#8221;20px||-3px|&#8221; \/][et_pb_text _builder_version=&#8221;3.0.89&#8243; background_layout=&#8221;light&#8221;]<\/p>\n<p><strong>Data on Canadian wait times, and what percentage of patients are treated within the acceptable &#8220;benchmark&#8221; standard. Data obtained <a href=\"http:\/\/waittimes.cihi.ca\/\">from the Canadian Institute for Health Information<\/a>.<\/strong><\/p>\n<p>[\/et_pb_text][et_pb_text _builder_version=&#8221;3.0.89&#8243; background_layout=&#8221;light&#8221;]<\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">Bacchus Barau, the associate director of health policy studies at the Fraser Institute, explains that one of the major reasons Canadians might seek treatment abroad is long wait times in the Canadian health care system.<\/span> <span style=\"font-weight: 400; font-size: medium;\">Barau notes that wait times vary within the country, with Nova Scotia and New Brunswick being some of the worst provinces to wait, and Quebec and Ontario being some of the best.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">According to Ontario&#8217;s Ministry of Health and Long-Term Care, the average Ontario patient stayed in emergency for 5.3 hours in January 2018. The average patient waited 63 days between being referred and their first surgical appointment.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">But Barau said in a phone interview that, as a whole, Canada has remarkably long wait times.<\/span> <span style=\"font-weight: 400; font-size: medium;\">According to a Fraser Institute report authored by Barua, in 2017, Canadian health care wait times were the highest in over 20 years. The average wait time was 21.2 weeks between referral and necessary treatment.<\/span> <span style=\"font-weight: 400; font-size: medium;\">Barau notes that other countries around the world with universal health care, such as Switzerland and Germany, still have better wait times than Canada. <\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">\u201cCanada is routinely at the bottom of the pack,\u201d he says.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b>Flying Blind<\/b><\/h3>\n<p><span style=\"font-size: medium;\"><span style=\"font-weight: 400;\">The wealth differences between Canada and most destination countries has led to<\/span><span style=\"font-weight: 400;\">\u00a0concerns<\/span><span style=\"font-weight: 400;\">\u00a0about lack of informed consent, which is when a medical tourist doesn&#8217;t completely understand the risks, benefits, or effects of a potential treatment. Without informed consent, Canadians might not fully understand what they&#8217;re getting into by becoming medical tourists.<\/span><\/span> <span style=\"font-size: 16px;\">Many Canadians go through medical tourism agencies to book their trips, but Snyder says Canadians should be wary of these agencies, which are biased.<\/span><\/p>\n<p>[\/et_pb_text][et_pb_video src=&#8221;https:\/\/www.youtube.com\/watch?v=qI5-lo0fW28&#8243; _builder_version=&#8221;3.0.89&#8243; max_width=&#8221;60%&#8221; custom_margin=&#8221;20px||-3px|200px&#8221; \/][et_pb_text _builder_version=&#8221;3.0.89&#8243; background_layout=&#8221;light&#8221;]<\/p>\n<p style=\"text-align: center;\"><strong>An advertisement video for a medical tourism agency in Ukraine.<\/strong><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">\u201cOn the clinic websites they\u2019re extremely positive, they\u2019re much more in the style of advertising,\u201d Snyder explains. \u201cVery few references to any risks or concerns that might be attached to medical tourism.\u201d<\/span> <span style=\"font-weight: 400; font-size: medium;\">He says he has also heard from patients who were promised a discount on their treatment by a clinic if they posted about their experiences.<\/span><\/p>\n<p>[\/et_pb_text][et_pb_video src=&#8221;https:\/\/www.youtube.com\/watch?v=C35KcZtQP4M&amp;t=94s&#8221; _builder_version=&#8221;3.0.89&#8243; max_width=&#8221;60%&#8221; module_alignment=&#8221;center&#8221; custom_margin=&#8221;20px||-3px|&#8221; \/][et_pb_text _builder_version=&#8221;3.0.89&#8243; background_layout=&#8221;light&#8221;]<\/p>\n<p style=\"text-align: center;\"><strong>A video testimonial by a medical tourist from Ottawa, ON, who received bariatric surgery in Cancun, Mexico.<\/strong><\/p>\n<p><span style=\"font-weight: 400;\"><span style=\"font-size: medium;\">If all the information that they&#8217;re getting is biased and one-sided, it can be difficult for Canadians to make a fully informed decision to become a medical tourist.<\/span><\/span> <span style=\"font-weight: 400;\"><span style=\"font-size: medium;\">Additionally, lack of intervention and regulation of medical tourism from the Canadian government can make the choice to travel abroad for care difficult.<\/span><\/span><\/p>\n<p><span style=\"font-size: medium;\"><span style=\"font-weight: 400;\">The federal government says that Canadians who choose to receive medical care abroad are \u201c<\/span><a href=\"https:\/\/travel.gc.ca\/travelling\/health-safety\/care-abroad\"><span style=\"font-weight: 400;\">on your own if you decide to do so<\/span><\/a><span style=\"font-weight: 400;\">,\u201d but warn of different health standards in other countries. The government also doesn\u2019t track the number of citizens who go abroad for treatment, or offer any follow-up care for patients.<\/span><\/span><\/p>\n<p><span style=\"font-size: medium;\"><span style=\"font-weight: 400;\">Snyder says if aftercare isn\u2019t in place, then a patient\u2019s success with a treatment might be worse.<\/span><\/span> <span style=\"font-size: medium;\">Even if a patient does receive aftercare upon returning to Canada, there are still potential problems.<\/span><\/p>\n<p><span style=\"font-size: medium;\"><span style=\"font-weight: 400;\">In a 2016 study of <\/span><a href=\"https:\/\/www.cmpa-acpm.ca\/en\/advice-publications\/browse-articles\/2016\/emerging-trends-and-medical-legal-risks-in-medical-tourism\"><span style=\"font-weight: 400;\">medical-legal risks of medical tourism<\/span><\/a><span style=\"font-weight: 400;\">, the Canadian Medical Protective Association notes patients often \u201creturn home with incomplete or no clinical documentation of information\u201d about their foreign procedure.<\/span><\/span> <span style=\"font-weight: 400; font-size: medium;\">Medical tourists may not fully understand or have documentation to explain their procedures and so they might not get appropriate care once back home, which can cause ripple effects in the Canadian health-care system and lead to even longer wait times.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b>Power Dynamic<\/b><\/h3>\n<p><span style=\"font-weight: 400; font-size: medium;\">Medical tourism doesn\u2019t just affect the tourist. It\u2019s also a concern for the country providing the care.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">Researchers have expressed concerns about the ethical dilemmas caused by high-income Canadians going to destination countries that are usually underdeveloped.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">International patients bring lots of money into these countries, and this might mean the foreigners are able to afford better private medical care than the locals who fund the public health care.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">\u201cThere\u2019s something problematic when taxpayers who paid for the development of that potential, the development of doctors and clinics, &#8230; \u00a0cannot access them because foreigners are getting them,\u201d Deonandan says.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">He notes that in reproductive tourism, where tourists sometimes go to lower-income countries for fertility treatments or surrogacy candidates, informed consent can be complicated.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">Not only do surrogates from less wealthy countries have less of a choice, as they\u2019re poor and need to make money, Deonandan also says the relationship between the doctor and the surrogate &#8212; their country, sex, age, and education &#8212; can all compel or coerce informed consent.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">\u201cThere\u2019s a power dynamic there that really complicates the soup.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">In some cases, the provincial government might actually help pay for medical expenses abroad.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">In Ontario, the Out of Country Prior Approval program provides funding for \u201cmedically necessary\u201d OHIP insured services while abroad, said David Jensen, media relations co-ordinator for the Ministry of Health and Long-Term Care, in an email.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">Jensen notes the program is not meant to provide access to international treatment when the same treatment exists within the province or when the treatment is not scientifically established, nor to enable queue-jumping.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">But even with this program in place, not every medical tourist qualifies. For those whose out of country treatment isn\u2019t funded, their money isn\u2019t going into the Canadian health-care system.<\/span><\/p>\n<p><span style=\"font-weight: 400; font-size: medium;\">Although many medical tourists pay out of pocket for their treatment abroad, when they return to Canada, any health problems they experience from post-treatment complications are now the burden of the Canadian health-care system.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b>Numbers Game<\/b><\/h3>\n<p><span style=\"font-size: medium;\">The fact there is no tracking of medical tourism by the federal government is part of the reason some researchers, including Snyder, are critical of the medical tourism data presented by the Fraser Institute.<\/span><\/p>\n<p><span style=\"font-size: medium;\">\u201cThere aren\u2019t any good numbers out there, so I guess you could say it\u2019s better than nothing but I\u2019m not sure that\u2019s true,\u201d Snyder says.<\/span><\/p>\n<p><span style=\"font-size: medium;\">He adds that although the study seems very precise, the numbers were self-reported by physicians. Since physicians don\u2019t record information on medical tourism, these data could be unreliable.
